Job Summary

We are seeking a skilled Nursing Specialist to join our team, supporting individuals with developmental disabilities and their families.


About the Role

This is a unique opportunity to make a meaningful impact in the community while promoting quality health care services. The successful candidate will be responsible for assessing and consulting on matters related to consumer growth and development, ensuring comprehensive health care provision.


Key Responsibilities
  • Participate in interdisciplinary planning teams and intake referrals for Early Start and over-age 3 consumers.
  • Assess and consult on consumer health needs, providing recommendations for nursing respite levels of care.
  • Collaborate with hospital discharge planners and monitor consumer health status post-discharge.
  • Provide consultation, training, and guidance on health-related issues to community providers serving the developmentally disabled population.

Requirements

To be considered for this role, you must be a Registered Nurse licensed under the State of California. You should also possess excellent communication skills and the ability to work effectively with diverse stakeholders.


Benefits

We offer a competitive salary range of $100,000 - $136,464 per year, as well as outstanding benefits, including CalPERS Retirement System, employer-paid medical insurance, and a hybrid work schedule after probationary period.
