Salary and Benefits
$40,000 Student Loan Repayment or $20,000 Sign-on Bonus for External Candidates

Job Description
In this role, you will be responsible for providing high-quality patient care, collaborating with other healthcare professionals, and working towards the Quadruple Aim of improving patient outcomes, enhancing the patient experience, reducing healthcare costs, and improving the work life of healthcare providers.
You will work closely with patients, their families, and other healthcare professionals to develop personalized care plans, educate patients on disease management and wellness strategies, and advocate for their needs. Your expertise will be instrumental in promoting healthy behaviors, preventing hospital readmissions, and improving overall patient satisfaction.
Key Responsibilities:
• Conduct comprehensive assessments, develop, and implement patient care plans
• Collaborate with PCPs, specialists, and other healthcare professionals to ensure seamless care coordination
• Provide education and support to patients and their families on disease management, wellness strategies, and medication adherence
• Prescribe diagnostics and interventions as necessary to prevent unnecessary acute admissions
• Participate in quality improvement initiatives and contribute to the development of clinical best practices
Requirements
To be successful in this role, you will need:
• Completion of a Nurse Practitioner or Physician Assistant accredited program
• Current state RN/NP or PA license
• Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or National Commission on Certification of Physician Assistants (NCCPA)
• Access to reliable transportation, if applicable
Preferred Qualifications
