Licensed Practical Nurse

Job Summary:
People Inc. is seeking a skilled Licensed Practical Nurse to join our team. As a key member of our healthcare team, you will provide nursing care and support to individuals with developmental disabilities living in community-based homes.
Key Responsibilities:
- Assist with monitoring and coordinating the delivery of preventive and routine health care services.
- Adhere to and assist with ensuring facility compliance with all physician, NP, PA, etc. orders.
- Complete required nursing care documentation promptly.
- Assist with coordinating medical program needs with community services, medical providers, and other related agencies.
- Ensure adequate scheduling and completion of health-related exams, appointments, laboratory/diagnostic testing as ordered or required.
- Safely transport individuals to and from appointments if necessary.
- Assist with the monitoring of medication administration in the facility.
- Administer medications, treatments, and supplements as assigned.
- Communicate issues and carry out all directives of the overseeing RN and Nursing Administration.
- Participate as an active member of the Interdisciplinary Team.
- Maintain compliance with and assist with ensuring facility compliance with all applicable State/Federal regulations and agency policy and procedure.
- Maintain communication with Medical Liaison, Site Management Team, Service Coordinator, and other staff as necessary.
- Participate in various nursing workgroups as assigned.
- Provide nursing coverage to all People Inc. programs as directed by Nursing Administration.
- Perform CPR and Strategies for Crisis Intervention and Prevention (SCIP-R).
- Comply with all agency policies and procedures.
Requirements:
- Current NYS licensure to practice as a Licensed Practical Nurse.
- Valid Drivers License that meets agency policy.
- Ability to meet physical requirements of the position when performing the following: standing, walking, sitting, bending, stooping, squatting, kneeling, and climbing.
- Lifting requirements of 35 lbs.
- Ability to lift and transfer individuals manually and mechanically as required by prescribed program/plan.
- Ability to move, reposition, and place individuals in pieces of equipment.
- Physical agility and ability to react to emergency situations, including maintaining Strategies for Crisis Intervention and Prevention (SCIP-R) certification as per regulatory guidelines.
- CPR certification required.
