Job Summary

The Senior Healthcare Coordinator at Presbyterian Senior Living is responsible for overseeing the care provided to residents in our Personal Care setting. This role involves supervising nursing staff, administering medications and treatments, and ensuring the highest level of quality care.

Responsibilities
  • Develop and implement individualized care plans in collaboration with healthcare teams
  • Coordinate and administer medications and treatments as ordered by physicians
  • Conduct regular resident assessments to identify changing needs and preferences
  • Collaborate with interdisciplinary teams to promote a culture of excellence in care delivery
  • Maintain accurate and comprehensive records of resident care and services provided
Requirements
  • A minimum of one year experience working in a Geriatric or Long-Term Care setting
  • Active Registered Nurse licensure required; valid CPR certification also necessary
  • Ability to communicate effectively with residents, families, and healthcare professionals
  • Familiarity with Electronic Medical Records (EMRs) and other health information systems
