Job Overview

This position involves providing high-quality Speech-Language Therapy services to patients in the comfort of their own homes. As a key member of our care team, you'll be responsible for assessing patients' communication and swallowing abilities, developing personalized plans of care, and delivering evidence-based interventions.

Your expertise will enable patients to safely remain in their homes, receive the support they need, and improve their overall quality of life.

Responsibilities
  • Conduct comprehensive assessments of patients' physical, functional, cognitive, and communication levels
  • Develop and implement individualized plans of care with measurable goals
  • Provide direct intervention and therapy sessions in patients' homes
  • Maintain effective communication and collaboration with patients, families, and healthcare teams
  • Educate patients and caregivers on strategies for improving communication and swallowing skills
  • Document all interactions, progress, and outcomes in a timely and accurate manner
Requirements
  • Active State Speech-Language Pathologist License
  • Current CPR/BLS certification
