**Job Summary**: As a Nurse Practitioner (NP) at Abode Care Partners, you will play a vital role in our team, providing compassionate and evidence-based medical care to our patients. You will be responsible for examining and treating various medical conditions, collaborating with healthcare professionals, and maintaining accurate patient records.
**Key Responsibilities**:
- Provide comprehensive medical care to seniors and patients with complex needs
- Conduct thorough examinations and develop treatment plans tailored to each patient's needs
- Collaborate with caregivers and other healthcare professionals to ensure seamless care coordination
- Maintain accurate and up-to-date patient records
- Order and interpret laboratory and diagnostic testing as needed
- Referrals to other healthcare practitioners when necessary
**Requirements**:
- Active state NP license
- Current NP National Board Certification
- Active state DEA licensure
- Master's degree in Nursing (MSN)
- Five or more years of nursing experience; at least two years of NP experience preferred
- Strong interpersonal communication skills – verbal and written
- Excellent time management skills with accountability for practitioner's daily workflow
- Proficiency in medical instruments and equipment
- Strong knowledge base of HIPAA and OSHA regulations
- Demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications
- Licensed driver with valid automobile insurance
- Ability to engage in moderate physical activity
