Job DescriptionPosition Summary:

The LPN Lead is responsible for assisting physicians and staff with daily patient care coordination and physician access. This role serves as a liaison between patients, staff, and physicians, ensuring seamless communication and efficient patient flow.

Essential Job Functions:
  • Provide professional nursing care to office patients in non-complex situations under the supervision of a physician and clinical practice manager or clinical coordinator.
  • Deliver direct patient care within the scope of licensure and in accordance with SMG policy.
  • Assume accountability for the quality of care delivered, including maintaining standard precautions.
  • Collect lab specimens according to departmental guidelines and accurately label them.
  • Prioritize patient care by implementing physician orders and nursing procedures.
  • Document patient information accurately in the Electronic Health Record (EHR).
  • Ensure all patients are correctly identified prior to providing and/or documenting care.
  • Communicate and provide care consistent with the patient's age, cultural, spiritual, and developmental needs.
  • Maintain efficient patient flow and employ appropriate use of Tasking in EHR.
  • Schedule appointments accurately according to department guidelines.
  • Administer injections according to protocol and document them accurately in EHR.
  • Effectively communicate problems, concerns, or issues to the Office Supervisor/Manager promptly.
  • Ensure proper preparation for patient visits and prepare pertinent information needed for the visit.
  • Obtain vital signs (BP, temp, pulse, height, and weight) accurately and document them in EHR.
  • Review and update medication lists in EHR, demonstrating understanding of prescription control and refill procedures.
  • Accurately document known allergies in EHR.
  • Enter orders and schedule appointments in the Practice Management System (PMS) and EHR.
  • Provide referrals and/or consult orders appropriately and in a timely manner in PMS and EHR.
  • Adhere to universal safety precautions when administering medications and disposing of medical waste.
  • Maintain proficiency with department-specific equipment, troubleshoot, and report malfunctions promptly.
  • Facilitate transition of care to UCC/Hospital.
  • Check and maintain logs for emergency equipment according to policy.
  • Assist in ordering and maintaining medical and office supplies as assigned.
  • Demonstrate flexibility with various work schedules.
  • Recognize and perform duties that need to be performed although not directly assigned.
General Job Functions:
  • Responsible for maintenance of staff and provider schedules in Shift Hound, working with the Office Supervisor/Clinical Coordinator to provide adequate staff coverage.
  • Responsible for oversight of medical and office supplies and ordering of supplies.
  • Monitor employee performance of staff assigned to the office, discussing concerns and issues with the Office Supervisor/Clinical Coordinator/Clinical Practice Manager.
  • Responsible for addressing and resolving patient complaints/issues, communicating issues and resolution to Providers, Office Supervisor/Clinical Coordinator/Clinical Practice Manager, and bringing concerns expressed by providers to the Office Supervisor/Clinical Practice Manager.
  • Notify the Office Supervisor/Clinical Coordinator/Clinical Practice Manager of any potential health and safety hazards in the office, responsible to place and follow through on Facilities/IT work orders related to the office setting.

Chemicals, chemotherapy, and fumes. Loud noises. Vibration. Extreme temperatures. Confined spaces. Allergens: dust, mold, and/or pollen. Magnetic fields. Radiation. Sharps. Latex. Blood-borne Pathogens: Exposure to infectious hazards, blood, body fluids, non-intact skin, or tissue specimens.

Education, Certification, Computer, and Training Requirements:

Vocational/Technical School/Diploma Program required. Associates degree preferred. New Jersey State Nursing License and BLS certification required. 0-1 years' experience. 2-4 years' experience preferred. Ability to communicate in English, both orally and in writing. Strong interpersonal and organizational skills. Ability to prioritize and organize multiple tasks in a timely manner. Experience with Standard Office Equipment (Phone, Fax, Copy Machine, Scanner, Email/Voice Mail) preferred. Experience with Standard Office Technology in a Window-based environment preferred. Experience with patient care equipment preferred. Experience with EHR, Practice Management System, and Departmental/Ancillary Systems preferred.
