About the Role

We are seeking an experienced and compassionate Care Manager to join our team at Sunrise Senior Living. As a Care Manager, you will be responsible for providing exceptional care and support to a consistent group of residents in our assisted living and reminiscence neighborhoods.

Job Description

Responsibilities:

  • Build meaningful relationships with residents and their families through regular communication and personalized care planning.
  • Develop and implement Individualized Service Plans (ISPs) to meet the unique needs and preferences of each resident.
  • Provide assistance with activities of daily living, life skills, and other life-enriching activities that promote independence and engagement.
  • Collaborate with the community team to ensure compliance with OSHA regulations and Risk Management policies.
  • Respond to resident needs and provide exceptional customer service to ensure a positive experience.
Requirements and Qualifications

To be successful in this role, you must possess the following qualifications:

  • Dedication to serving seniors with excellent customer service skills.
  • Positive attitude, flexibility, and ability to work in a team environment.
  • High School diploma or equivalent required; certifications may be necessary in certain states.
  • Previous experience working with seniors is preferred.
  • Able to make choices, decisions, and act in the best interest of residents.
  • Competent in written and verbal communication, organizational, and time management skills.
