Appalachian Regional Healthcare is seeking a skilled Care Manager I to join our team. As a Care Manager I, you will be responsible for coordinating the care and services of selected patient populations across a continuum of care.
Key Responsibilities:
- Coordinate and collaborate with physicians, providers, and multidisciplinary teams to ensure optimal quality, clinical, and cost outcomes.
- Develop, implement, and maintain work standards and procedures that comply with ARH's policies and governmental regulations.
- Assess, develop, implement, and monitor comprehensive plans of care through an interagency multi-disciplinary team process.
- Revise and adjust plans of care daily to accommodate the needs of individual patients.
- Assume responsibility and accountability for the care plan and effectiveness of patient outcomes.
Requirements:
- BSN preferred with 5 years of experience in a hospital/community setting.
- Or RN with MSN or degree in other related field with 3 years of experience.
- Must meet all licensure and certification requirements in the state working.
- Excellent oral and written communication skills.
- Proven organizational skills.
