Job Summary

We are seeking a skilled Medical and Direct Care Professional to join our team at Community Residences. This dynamic role combines medical care with direct support services, providing a unique opportunity for individuals to grow and thrive in a supportive environment.

Responsibilities
  • Administer medications at prescribed times, adhering to medication administration policies and documenting accordingly.
  • Monitor medical, physical, and behavioral changes in individuals, using the DAR (Data-Action-Response) formula for charting and reporting as required.
  • Maintain individuals' right to privacy and adhere to CRI's confidentiality policy.
Duties
  1. Create a nurturing, positive learning environment and develop strategies to help individuals cope effectively with daily living situations.
  2. Implement Individual Plans of Service, teaching strategies, and guidelines based on interdisciplinary team identification of individual needs.
  3. Assist with meal preparation, feeding, cleaning up after meals, documenting intake, and purchasing groceries and supplies.
  4. Maintain the cleanliness of the home, inside and outside, and assist with laundry care.
Requirements
  • Must possess a valid CT driver's license, be able to drive own and agency vehicles, and have a good driving record.
  • Must have reliable transportation.
  • Must have one year experience working as an LPN or have successfully completed CRI's LPN-in-training program.
Qualifications
  • Possess good to excellent oral and written communication skills.
  • Demonstrate thoroughness and follow-through on daily work assignments.
