We are seeking an experienced Home Health Therapist to join our team at LHC Group. This role offers a unique opportunity to make a meaningful difference in the lives of patients and families in the Atmore, Alabama area.

About the Role

The successful candidate will be responsible for assessing patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, they will develop a treatment plan, perform interventions aimed at improving and enhancing patient well-being, and evaluate patient progress.

  • Evaluate patient functional status and physical therapy needs, assisting physicians in developing therapy plans of care.
  • Observe and report patient responses to treatment and any changes in condition to supervising nurses and/or physicians.
  • Treat patients to relieve pain, develop, and restore function, or maintain maximum functioning using acceptable standards of practice.
  • Instruct and inform healthcare team members, patients, and family/caregivers regarding plans of care, functional limitations, and progress toward goals.

This is a full-time position offering competitive compensation and benefits, including $58,000 per year, holiday pay, medical benefits, dental benefits, vision benefits, 401(k) retirement plan, continuing education opportunities, employee assistance programs, and a sign-on bonus.

Required Skills and Qualifications

To be successful in this role, candidates must possess:

  • A current Physical Therapy licensure in the state of practice
  • A valid CPR certification
  • A current driver's license and vehicle insurance, with access to dependable transportation
