The Role

  • Welcome patients to the benefits of remote monitoring, empowering them to take control of their health.
  • Educate patients on equipment usage, ensuring seamless integration into their daily lives.
  • Near real-time review of patient vital signs, allowing for swift intervention when needed.
  • Obtain actionable information from patients with abnormal readings, facilitating informed decision-making.
  • Utilize practice EMR to escalate patient situations to the provider for medical intervention, ensuring timely treatment.
  • Review and assist providers in creating patient-centered care plans, promoting holistic care.
  • Monitor notifications and observe trends, identifying potential issues before they become major concerns.
  • Create patient-centered care plans and review monthly plans with patients, fostering ongoing engagement.
  • Monitor patient compliance with the plan of care and devices, encouraging adherence.
  • Counsel patients on healthier behaviors, promoting long-term wellness.
  • Act as the first point of contact for patient motivational factors, leveraging them to drive optimal health outcomes.
  • Proactively manage patients to prevent urgent care or emergency room visits, reducing healthcare costs.

Requirements

  • 2 years of nursing experience, with case management or home health experience preferred.
  • Active state licensure (compact licensure preferred).
  • EMR experience.
  • Computer competency.
  • Experience caring for conditions such as hypertension, diabetes, COPD, asthma, CHF, and atrial fibrillation.

Job Details

  • Full-time, salaried position.

We offer a competitive salary range of $65,000 - $90,000 per year, depending on experience.
