Case Manager Specialist

As a Case Manager Specialist with the County of Douglas CO, you will be responsible for performing full performance level intensive case work services including case management, referral, placement, and assessment/evaluation. This role requires independent judgment and a significant understanding and application of professional principles and practices.
Key Responsibilities:
Conduct thorough assessments and evaluations of clients to determine their needs and develop effective case plans.
Provide case management services, including referral and placement, to ensure clients receive the necessary support and resources.
Collaborate with other professionals, such as social workers and healthcare providers, to ensure comprehensive care for clients.
Develop and maintain accurate and detailed records of client interactions and case progress.
Requirements:
Bachelor's degree in a related field, such as social work or psychology.
2+ years of experience in case management or a related field.
Strong communication and interpersonal skills.
Ability to work independently and as part of a team.
