Job Summary:

We are seeking a Nurse Practitioner or Physician Assistant to join our team of healthcare professionals for home risk assessments. In this role, you will work collaboratively with our clinicians to review patient health, medical history, medication adherence, social environment, and other risks. Your expertise will be instrumental in identifying potential care gaps and developing personalized care plans to improve patient outcomes.

Responsibilities:

  • Conduct comprehensive adult/geriatric assessments, including medical history, diagnosis, and treatment
  • Develop and implement individualized care plans to address patient needs and priorities
  • Collaborate with primary care physicians, case managers, social workers, and other healthcare professionals to ensure seamless care coordination
  • Provide education and support to patients and their families on health management and self-care strategies
  • Document all interactions and interventions in accordance with Matrix Medical Network guidelines and protocols

Requirements:

  • Master's degree in Nursing or Physician Assistant Studies, or equivalent experience and licensure
  • Current RN and NP or PA licensure in the state of practice, with prescription authority
  • Board certification in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant, with a strong background in adult/geriatric care
  • Excellent communication and interpersonal skills, with the ability to work independently and as part of a team
