Role Overview

We are seeking an experienced LPN to join our team as a Senior Care Coordinator in our Assisted Living department.

Responsibilities

In this role, you will be responsible for creating personalized care plans for our residents, working closely with their loved ones and physicians to ensure seamless care delivery. You will also coach and support your colleagues to provide exceptional resident care, round with physicians to monitor medications, and educate residents and their families on health and wellness issues.

Requirements

To be successful in this role, you will need an LPN license in the state where employed, CPR certification, and one year of experience in a healthcare setting. Strong leadership skills, ability to form relationships with residents and their loved ones, and physical stamina to perform tasks required in this role are essential.

Compensation Package

This role offers a competitive salary range of $27.50-$30.50 per hour, based on shift preference, with opportunities to earn more based on experience. We also offer comprehensive benefits, including Highmark Blue Shield medical coverage, excellent vision and dental, 401(k) retirement plan with company match, paid time off, and other perks.
