**Job Description:The Patient Care Coordinator plays a critical role in providing personalized support to our residents. Key responsibilities include:
- Developing and implementing individualized care plans.
- Collaborating with interdisciplinary teams to ensure seamless resident care.
- Monitoring and documenting resident progress.
**Required Skills and Qualifications:**
- Minimum 1 year of experience in healthcare.
- CNA license required.
- Strong communication and interpersonal skills.
