  • Collaborating with interdisciplinary teams to ensure seamless resident care.
  • Monitoring and documenting resident progress.

**Required Skills and Qualifications:**

  • Minimum 1 year of experience in healthcare.
  • CNA license required.
  • Strong communication and interpersonal skills.
