About the Role

We are seeking a highly skilled and dedicated Registered Nurse to join our team as a Family Medicine Clinical Nurse Specialist. This role is responsible for providing exceptional care to patients in a clinic setting, working closely with our healthcare team to deliver high-quality patient outcomes.

Key Responsibilities

  • Provide direct patient care, including assessing, planning, implementing, and evaluating patient care plans
  • Collaborate with healthcare professionals to coordinate patient care and ensure continuity of treatment
  • Develop and implement education programs for patients and their families on health promotion, disease prevention, and management
  • Participate in quality improvement initiatives to enhance patient care and safety
  • Contribute to the development and implementation of clinical policies and procedures

Requirements

  • A graduate of a state-approved or National League of Nursing-accredited School of Nursing
  • RN license or eligible for licensure in the state of Delaware and Pennsylvania
  • Two years' experience as a Registered Nurse, with a focus on family medicine or pediatrics preferred
