Job Overview
We are seeking a skilled Mental Health Rehabilitation Professional to join our team at PermiaCare. In this role, you will work with individuals who require mental health services to assist them in their recovery and daily living.
About the Role
This position is an excellent opportunity for a dedicated professional to make a positive impact in the lives of our clients. You will be working in a community setting, providing rehabilitation services to adults with severe mental illness. Your goal will be to help reduce the risk of chronic diseases related to stress, anxiety, and substance abuse.
Key Responsibilities
- Provide rehabilitation services in the community as prescribed in the Medicaid Reimbursement Provider Manual.
- Work closely with clients on a regular basis, based on their needs.
- Ensure the quality of services through accurate documentation.
- Collaborate with team members to ensure that clients remain in a community setting.
- Inform eligible individuals about available services and how to access them.
- Provide education and coping skills to families of clients.
- Advocate for service delivery for at-risk clients and their families.
- Comply with Medicaid and PermiaCare documentation standards.
- Implement TRR and Recovery Plans.
- Enter services into EHR within given timeframes.
- Transport clients using personal vehicle as needed.
- Maintain an assigned caseload of individuals with mental illness.
- Coordinate services to designated caseload.
- Document services accurately and within timeframe required.
- Maintain confidentiality of sensitive records and treatment information, client files, and protected health information in compliance with HIPAA, laws, rules, and regulations, and established procedures.
- Attend to regular and reliable physical on-site attendance. Regular attendance, dependability, and punctuality are required for the scheduled workday 100% of the time to ensure consistency and completeness of program processes.
- Adhere to the Abuse, Neglect, and Exploitation policy and reporting requirements.
- Establish and maintain effective work relationships with individuals served and their families, supervisors, coworkers, and visitors by demonstrating cooperative, courteous, and respectful behavior at all times.
Requirements
- Bachelor's degree from an accredited college or university with a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistance, gerontology, special education, educational psychology, early childhood education, or early childhood intervention, or a bachelor's degree with at least 30 hours of coursework in these fields.
- At least one year of experience in the mental health field preferred.
- Valid Texas Driver's license, auto liability insurance, and an acceptable driving record are required.
- Certification, registration, or licensure: Must obtain QMHP certification within six months of hire.
Working Conditions
This position requires travel and may involve working in various environments, including offices and community settings. The ability to drive a personal vehicle and transport clients as needed is also required.
Estimated Salary Range
$20.65 per hour, with a sign-on bonus potentially available.
