Job Summary

Responsibilities

The Occupational Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment. Key responsibilities include:
  • Providing services within the scope of practice as defined by state laws governing the practice of Occupational Therapy, in accordance with the plan of care, using evidence-based techniques, and in coordination with other members of the health care team.
  • Evaluating the patient's functional status, status of all body systems as required for CMS documentation, and occupational therapy needs. Consulting with the physician in the development of the therapy plan of care.
  • Observing, recording, and reporting to the nurse supervisor and the physician the patient's response to treatment and changes to the patient's condition.

Requirements

To be successful in this role, you will need to have a current Occupational Therapy licensure in the state of practice, current CPR certification, and a current driver's license, vehicle insurance, and access to a dependable vehicle, or public transportation.
