We are seeking a highly motivated and compassionate Mentorship Specialist to join our team at Envision Unlimited. This is an exciting opportunity to work with individuals with disabilities, providing support and guidance as they navigate their journey towards independence.

About the Role:

  • As a Mentorship Specialist, you will be responsible for providing individualized support to consumers in a variety of settings, including community-based programs, residential facilities, and hospitals.
  • You will work closely with consumers to develop personalized goals and plans, empowering them to achieve their aspirations and improve their overall quality of life.
  • Your role will involve building strong relationships with consumers, families, and healthcare professionals, ensuring a seamless transition from hospital to community-based care.

Key Responsibilities:

  • Develop and implement individualized support plans tailored to each consumer's unique needs and goals.
  • Provide education and technical assistance to consumers and families on topics such as daily living skills, medication management, and health promotion.
  • Collaborate with healthcare professionals to ensure comprehensive care and coordination.
  • Conduct regular progress reviews and assessments to evaluate the effectiveness of support plans.
  • Document all interactions, progress, and outcomes in accordance with agency policies and procedures.

Requirements:

  • Bachelor's degree in a related field (e.g., social work, psychology, counseling) or equivalent experience.
  • Minimum 1 year of experience working with individuals with disabilities, preferably in a mental health or community-based setting.
  • Strong communication and interpersonal skills, with ability to build rapport with diverse populations.
  • Knowledge of community resources, mental health services, and treatment options.
  • Familiarity with Medicaid billing and reimbursement processes.

What We Offer:

  • A competitive salary of $45,000 - $55,000 per annum, commensurate with experience.
