Necnic Group is seeking an experienced Family Medicine Healthcare Provider to join our team in Philadelphia, PA.

The estimated salary for this role is $120,000 - $180,000 per year, depending on experience.

Job Description:

We are looking for a skilled healthcare provider to provide comprehensive primary care to patients with chronic and complex illnesses. As a Field-Based NP, you will work closely with an interdisciplinary team to improve patient outcomes and reduce unnecessary healthcare costs.

Key Responsibilities:

  • Conduct in-home visits for patients enrolled in the program, performing comprehensive evaluations and follow-up visits targeting chronic disease management and prevention.
  • Prescribe medications, order diagnostic tests, and coordinate care with the patient's PCP and other specialists.
  • Educate patients and their families on chronic conditions, preventative care, and medication adherence.
  • Collaborate with an interdisciplinary team, including social work, nursing, pharmacy, palliative care, and behavioral health professionals.
  • Document patient encounters accurately in the EMR system and participate in virtual clinical meetings and training sessions.
  • Share on-call coverage with other team members, including limited weekend responsibilities.

Qualifications:

  • Graduate of an Advanced Nurse Practitioner program in Adult, Geriatric, or Family Medicine.
  • Active, unrestricted NP license in Pennsylvania.
  • Minimum 1 year of NP experience in primary care, geriatrics, adult medicine, or home care.
  • Current BLS certification required; ACLS preferred.
  • DEA license and State Controlled Substance License upon hire or within 3 months.
  • Valid state driver's license and proof of yearly influenza, Hepatitis B vaccinations, and TB testing.
