Speech Language Pathologist Home Health Per Diem Position

About the Role:
CenterWell Home Health is seeking a skilled Speech Language Pathologist to join our team. As a Speech Language Pathologist, you will play a vital role in providing personalized care to patients in their homes.
Key Responsibilities:
- Evaluate and direct speech/language pathology services to patients in the home or facility
- Participate in the development and periodic review of the Plan of Treatment and Plan of Care
- Administer and interpret diagnostic tests and applications of therapeutic treatments
- Observe, record, and report changes in the patient's condition and response to treatment
- Provide instruction and training to patients in the use of alternative communication systems
- Provide counsel and instruction to patients, families, and healthcare staff
- Maintain and submit documentation as required by the Company and/or facility
Requirements:
- Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA
- Minimum of six months experience as a speech therapist/speech language pathologist
- Home Health experience a plus
- Current and unrestricted license
- Current CPR certification
- Good organizational and communication skills
Benefits:
CenterWell Home Health offers a competitive compensation package, including a pay range of $84,500 - $116,400 per year, as well as benefits such as paid time off, 401(k) retirement savings plan, employee assistance program, business travel and accident insurance.
