Nurse Case Manager

Las Palmas Medical Center is seeking a skilled Registered Nurse Case Manager PRN to join our team. As a key member of our healthcare team, you will play a vital role in delivering excellent care to our patients.
About the Role:- Coordinate the delivery of medical services and discharge planning for a specified patient population.
- Conduct concurrent and retrospective review of patient medical records for utilization review, compliance, and quality assurance.
- Develop and maintain cooperative relationships with patients, families, facility staff, physicians, and community agencies.
- Ensure continuity in the handoff of patient clinical information from the hospital to other healthcare entities.
- Three to five years of clinical experience in an acute healthcare setting, with recent work history in acute care case management or related healthcare experience.
- Working knowledge of case management philosophy, needs assessment, utilization review, quality assurance, and discharge planning.
- Ability to analyze complex clinical and financial data and apply it to level of care criteria and discharge options.
- Strong clinical knowledge and understanding of disease processes.
- Computer literacy and familiarity with basic office equipment.
- Effective communication skills with hospital personnel, physicians, patients, and outside agencies.
- Ability to communicate effectively with patients of various age groups.
