The Role:
As an HIV Nurse Case Manager at Wayne Health, you will play a critical role in helping individuals living with HIV manage their medical care needs and achieve optimal patient outcomes. You will work closely with patients, medical providers, and other healthcare professionals to understand their medical and psychosocial needs and develop personalized care plans.

Key Responsibilities:
  • Assist clients living with HIV to enroll in medical insurance, Medicaid, and/or MIDAP.
  • Ensure that all eligible funding sources are tapped before accessing Ryan White funds.
  • Enroll eligible patients into the Ryan White program and case management program, adhering to current Ryan White Standards of Care.
  • Conduct comprehensive care plans for enrolled clients, assessing needs and support systems, and providing resources to eliminate or reduce barriers to treatment.
  • Monitor client progress, reassess needs, and revise care plans as necessary.
  • Provide education and counseling on medication adherence and refer clients to AIDS and social service agencies as needed.
  • Document client data, units of service, and care plans in a timely manner.

Requirements:
  • Azure Degree in Nursing (Associate Degree)
  • Bachelor's of Science Degree in Nursing preferred
  • Current Michigan Registered Nurse License
  • Basic Life Support (BLS) Certification
  • 1-2 years of experience in ambulatory care settings preferred
  • Knowledge of nursing theory and practice

Estimated Salary Range:
$65,000 - $85,000 per year, depending on experience and qualifications.
