Job Description

At Institute on Aging, we are committed to providing innovative healthcare solutions that empower individuals to maintain their quality of life. As a Care Manager II, you will play a vital role in assessing and planning for the well-being of our clients with complex medical and psychosocial needs.

Responsibilities
  • Comprehensive Assessments: Conduct thorough assessments of clients, including psychosocial, physical, and mental health, environmental, and spiritual needs.
  • Community Living Plan: Develop and initiate client-centered, comprehensive, and consistent plans with program guidelines and policies.
  • Home Visits and Escort Services: Conduct home visits, acute hospital, and skilled nursing facility visits, as well as escort clients to medical and other appointments as clinically indicated.
  • Community Services: Identify, arrange for, and monitor appropriate community services based on a good knowledge of Medicare, Medi-Cal, and other entitlement programs.
  • Care Management Relationship: Establish and maintain a care management relationship with clients and their informal support network, offering respect, dignity, and support.
  • Documentation: Document all case management activity regarding identified problems within 24-48 hours, adding any new problems to the Community Living Plan as needed.
  • Communication: Establish and maintain open and effective communication with community providers, including physicians and other healthcare and social service workers.
Requirements
  • Education: Master's in Social Work or related field required.
  • Experience: One year working with disabled adults and/or older adults required.
  • Skills: Exceptional communication and presentation skills, case management skills, and experience in the community healthcare delivery system.

Compensation: $75,000-$86,000 per year. This range is not necessarily reflective of actual compensation, which may be dependent on experience, education, and other factors.
