About the RoleThis is a field-based position with a home-based office, requiring local travel up to 50% of the time. You will work from home when not in the field.
Primary Responsibilities- Care Coordination: Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care.
- Community Outreach: Utilize both company and community-based resources to establish a safe and effective case management plan for members.
- Patient Advocacy: Collaborate with patients, families, and healthcare providers to ensure quality coordinated care and services are provided expeditiously to all members.
- Competitive Salary: $65,000 - $85,000 per year, depending on experience.
- Comprehensive Benefits Package: Including medical, dental, vision, and life insurance, as well as a 401(k) savings plan and employee stock purchase plan.
- Paid Time Off: Plus 8 paid holidays per year.
- 4+ Years of Experience: In case management, community outreach work, or volunteering experience.
- Intermediate Level of Computer Proficiency: Including Microsoft Outlook, Teams, and multiple web applications.
- Valid Missouri or Kansas Driver's License: And current automobile insurance with access to reliable transportation.
