Our Hospice Care team takes a proactive approach to pain and symptom management, addressing the physical, emotional, and spiritual needs of patients, caregivers, and families. We strive to provide a comfortable, dignified, and respectful experience for all.

As a member of our team, you will work closely with a multidisciplinary team to provide comprehensive care and support, empowering patients to take control of their lives and treatment. You will be responsible for delivering high-quality, patient-centered care, adhering to our organizational professional practice model and competencies.

**Responsibilities:**

  • Provide safe, effective, and compassionate care to patients and their families, utilizing evidence-based practices and research.
  • Collaborate with the interprofessional healthcare team to facilitate coordination of care across the continuum.
  • Participate in Continuous Quality Improvement initiatives to identify and address potential and actual problems.
  • Engage in systematic, evidence-based problem-solving and decision-making to promote effective changes within the care delivery system.
  • Recognize patients as the source of control and a full partner in providing compassionate and coordinated care.
  • Minimize risk of harm to patients, families, providers, and self through system effectiveness and individual performance.
  • Utilize appropriate information and technology to communicate, manage knowledge, mitigate error, and support decision-making.
  • Engage in the process of cooperation, coordination, and collaboration to provide safe, quality outcomes for patients within inter and intra-professional teams.

Credentials:

  • Registered Nurse - PA
  • Driver's License (Required)
  • Basic Cardiac Life Support (Required)
  • Car Insurance (Required)

Education or Equivalent Experience:

  • Associates or Bachelor of Arts or Science (Required)
  • Education Specialization: ASN or BSN from an accredited school of nursing
  • Equivalent Experience: More than 15 months relevant professional nursing experience and completion of the Nurse Residency Program, if applicable
