We are seeking a skilled and compassionate Clinical Nurse Practitioner to join our team at Matrix Medical Network. As a key member of our clinical staff, you will be responsible for conducting comprehensive health assessments in the home setting.

About the Role:

This is a PRN/Part-Time opportunity that requires flexibility and adaptability. You will work independently to assess patients' health needs, develop care plans, and provide education to patients and their families. Your expertise as a Nurse Practitioner will be invaluable in helping us improve quality of care and address chronic conditions.

Key Responsibilities:
  • Conduct comprehensive health assessments in the home setting
  • Develop and implement individualized care plans
  • Provide education to patients and their families on disease management and prevention
  • Collaborate with healthcare providers to ensure seamless care transitions
Requirements:
  • Master's Degree in Nursing or commensurate experience and licensure
  • Current RN and NP licensure with prescription authority
  • Board certification in a relevant specialty
  • Minimum 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
