Geriatric Care Specialist for Outpatient Program in New Mexico

We are seeking a skilled Geriatric Care Specialist to join our interdisciplinary team in Albuquerque, NM. As a Primary Care Locums Opportunity, you will have the chance to work with a dynamic and collaborative group of healthcare professionals.
About the PositionThis is a full-time or part-time opportunity with a guaranteed minimum of 90 days, with the possibility of extension or permanent conversion. You will be working in an outpatient setting, providing high-quality care to patients aged 65 and older. The ideal candidate will have excellent communication skills, a strong understanding of geriatric medicine, and the ability to work collaboratively as part of a team.
Key Responsibilities- Providing comprehensive geriatric care to patients, including assessments, diagnoses, and treatment plans
- Collaborating with other healthcare professionals, including dentists, social workers, psychiatrists, and mid-levels, to ensure continuity of care
- Conducting daily meetings with the interdisciplinary team to discuss patient care plans and progress
- Participating in the development and implementation of patient-centered care plans
- Board Certified or Board eligible in Geriatric Medicine
- Active New Mexico medical license
The compensation for this position is negotiable, based on experience and qualifications. We also offer additional benefits, including:

Estimated Salary: $175-$225 per hour
