Job Summary

We are seeking a dedicated and compassionate Home Health Physical Therapist to join our team. As a member of our interdisciplinary care team, you will play a vital role in delivering high-quality patient care services and enhancing the lives of our patients and their families.

About the Role
  • This full-time position offers a competitive salary package, including an estimated $85,000 annual base pay, plus benefits and a sign-on bonus.

In this role, you will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on your assessment and evaluation, you will develop and implement treatment plans aimed at improving and enhancing the patient's well-being.

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively as part of a multidisciplinary team. You will also have strong clinical skills and experience working with patients of varying ages and abilities.

Key Responsibilities
  1. Evaluate patients' functional status and physical therapy needs, assisting physicians in developing personalized therapy plans of care.
  2. Observe and report patients' responses to treatment and any changes in their condition to supervising nurses and/or physicians.
  3. Treat patients to relieve pain, develop and restore function, and maintain maximum functioning using accepted standards of practice.
  4. Instruct and inform participating members of the healthcare team, patients, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.

To be successful in this role, you must hold a current Physical Therapy licensure in the state of practice, possess CPR certification, and have access to a dependable vehicle.
