About the Role

As a Primary Care Physician Assistant at Monogram Health, you will be responsible for conducting comprehensive annual wellness exams, counseling and educating patients and families about benefits and programs available to help them live healthier lives, and documenting items such as chief complaint, diagnosis, past medical history, family and social history, review of systems, examinations, medications, allergies, assessment, and plan. You will also coordinate care with primary care providers, specialists, and ancillary services, complete all documentation and paperwork in a timely manner, and maintain quality of care standards defined by the practice.

Responsibilities

Requirements

To be successful in this role, you must possess a Bachelor's degree in Nursing or related field, and be certified as a Nurse Practitioner or Physician Assistant. You should also have a valid driver's license, access to transportation, and be proficient in medical instruments and equipment required by the work. Additionally, you should have experience conducting annual wellness visits or similar comprehensive visits virtually or in the home, and be able to communicate effectively in verbal and written form with retail and medical partners, patients, family members, physicians, and representatives of the community.

Salary and Benefits

We offer a competitive salary of $125,000 per year, plus benefits including medical, dental, vision, and life insurance, paid vacation and holiday time, 401(k) plan with matching contributions, and paid relocation assistance.
