About the Role

As a Private Duty LPN Home Care Specialist, you will be responsible for providing direct nursing care to patients in their homes. You will work independently, making decisions based on your assessment of the patient's needs, and collaborate with other healthcare professionals to ensure high-quality patient care.


Job Responsibilities
  • • Provide high-quality clinical services within the scope of practice, following all applicable infection control procedures.
    • Document patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record.
    • Actively participate in the agency's care coordination process, including timely reporting patients' needs and changes in condition.
    • Ensure confidentiality of patient health information per agency policy.

Requirements
  • • Current LPN licensure in the state of Arkansas without restrictions.
    • At least one year of experience working as a LPN, preferably in home care or a similar setting.
