Salary: $83,000 - $110,000 per year, depending on experience

Job Summary

The Healthcare Social Worker - Acute Care Specialist will be responsible for providing high-quality patient care and support services to patients and their families. This role requires excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Responsibilities:

  • Conduct comprehensive assessments of patients' social and emotional needs
  • Develop and implement effective care plans to address patients' needs
  • Provide education and support to patients and their families
  • Collaborate with interdisciplinary teams to ensure seamless care delivery
  • Maintain accurate and timely documentation of patient interactions and outcomes
Requirements

To be successful in this role, you will need:

  • A Master's degree in Social Work (MSW) or related field
  • Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) certification
  • At least 2 years of experience in acute care social work
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
