Transforming Healthcare for the Medically Vulnerable

Landmark Health, a part of Optum, is seeking an Advanced Practice Clinician to join our team in providing home-based medical care to chronically ill patients. Our medical group is dedicated to transforming how healthcare is delivered to the most medically vulnerable members in our community.

Key Responsibilities:

  • Provide medical care and management to members served by the program
  • Participate in gathering clinical information for medical necessity determinations for service requests
  • Collaborate with the Landmark Health Medical Director, Director of Health Services, and UM staff in the development and/or implementation of medical management policy, clinical protocols, utilization management guidelines, and quality management programs
  • Establish and maintain effective relationships with members and their legal representatives, facility staff, PCPs, and other collaborating physicians/providers

Requirements:

  • Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA)
  • ~2+ years of work experience applying the knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems
  • Proven ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
