We are currently looking for an experienced human/social services professional to support individuals with intellectual and developmental disabilities at Connections For Life. Our Care Coordinators work in various settings, offices, communities, and individuals' homes, providing basic daily services to help individuals develop personal goals and decision-making skills.

As a Care Coordinator Specialist, you will supervise and develop a team of staff assigned to their caseload to coordinate services so individuals are healthy and safe, and have their personal care needs met (e.g., hygiene, grooming, dress), and engage in preferred leisure-time and social activities.

The estimated salary for this role is $31-$41 per hour, based on industry standards in the location.

Key Responsibilities:
  • Supervise and develop a team of staff assigned to their caseload to coordinate services
  • Monitor health and medical needs and understand the POS status of each individual
  • Provide support and training in home, work, mobility, and social/recreational environments
  • Conduct house checks and complete documentation
  • Intervene in crisis situations and locate necessary resources
  • Assess and prepare quarterly reports for North Bay Regional Center regarding Individual Support and Program Plans

In addition to the core responsibilities, as-needed tasks may include ensuring prescribed medications are ordered and picked up in a timely fashion, coordinating support teams, and maintaining effective working relationships with team members, other providers, and family.
