Job Summary
We are seeking a skilled Registered Nurse to join our homecare team. The successful candidate will be responsible for assessing and evaluating patients, developing care plans, and collaborating with healthcare providers.
Key Responsibilities:
- Provide Skilled Nursing Care: Deliver high-quality nursing care to patients in their homes, including medication management, wound care, and vital sign monitoring.
- Patient Assessments and Evaluations: Conduct thorough assessments and evaluations of patients with multiple health conditions to develop personalized care plans.
- Care Plan Development: Collaborate with healthcare providers to develop and implement care plans that address the unique needs of each patient.
- Documentation and Communication: Accurately document patient information and communicate effectively with healthcare providers and multidisciplinary teams.
- Infection Control and Safety: Adhere to agency infection control and safety policies to ensure a safe environment for patients and staff.
Requirements
To be successful in this role, you will need:
- A Bachelor's degree in Nursing and current RN licensure.
- At least one year of supervised clinical experience as a registered nurse.
- Excellent verbal and written communication skills.
