Job Summary

As a Registered Nurse Case Manager, you will be responsible for coordinating patient care plans, managing length of stay, and developing safe discharge plans in collaboration with the multidisciplinary team. You will facilitate patient progression and transition using established criteria, ensuring quality outcomes and optimal resource utilization.

  • You will provide case management services for inpatient and observation patients as assigned.
  • You will identify patients at risk for adverse outcomes during transitions between care levels or settings.
  • You will perform comprehensive assessments of patients' psychosocial, medical, and discharge needs, as well as available resources.
Requirements

To succeed in this role, you will need:

  • 2+ years' experience in case management or 3+ years' experience in clinical nursing.
  • InterQual experience is preferred.
