OverviewLHC Group, a leading provider of post-acute care services, is seeking an experienced Licensed Practical Nurse to join our team as a Senior Care Nurse Specialist. As a key member of our home health care team, you will be responsible for providing exceptional patient care and support to individuals in need of skilled nursing services.

Responsibilities- Provide quality patient care and perform technical skilled care in the patient's home, ensuring compliance with state laws governing the practice of nursing and the plan of care.
- Complete progress notes for each visit and submit them to the agency daily, accurately documenting patient responses to treatment and changes in condition.
- Continuously observe and report patient nursing needs, instructing family members in patient care and maintenance of a healthy environment.
- Current LPN licensure in the state of practice.
- Valid Driver's License, vehicle insurance, and access to a reliable vehicle or public transportation.
- CPR Certification required.
