Job Title: Physical Therapist

Job Summary:
We are seeking a skilled Physical Therapist to join our team at AristaCare. As a Physical Therapist, you will be responsible for providing high-quality physical therapy services to residents in our In-House Skilled Nursing Facility.

Key Responsibilities:

  • Provide physical therapy services to residents in accordance with established standards of practice and company procedures.
  • Develop and implement individualized treatment plans to meet the unique needs of each resident.
  • Conduct regular screenings and evaluations to determine the need for intervention or treatment.
  • Communicate effectively with residents, families, and interdisciplinary team members to ensure seamless care.
  • Maintain accurate and compliant documentation in our NetHealth EMR system.

Requirements:

  • Bachelor of Science Degree in Physical Therapy from an accredited program.
  • Active Physical Therapy license in the state(s) of practice.
  • Ongoing continuing education and professional development to maintain current licensure and certification.
