Job Summary

We are seeking a highly skilled Licensed Practical Nurse 4 to join our team at the State of Washington. As an on-call LPN, you will provide direct resident care to vulnerable Veterans in need of skilled nursing care.

Key Responsibilities
  • Provide direct resident care, including monitoring and recording all aspects of care, passing medications, dressing changes, and treatment.
  • Update and maintain MDS documentation.
  • Evaluate, prepare, and revise treatment plans for individual residents based on personal observations and recommendations of physicians, psychiatrists, psychologists, social workers, and/or other nurses.
  • Assist with routine rehabilitative and maintenance care.
  • Assist residents with positioning, transferring, and ambulation.
  • Monitor and maintain a safe environment, including preventative infection control.
Requirements
  • Valid Washington State practical nursing license or unencumbered multistate license.
  • Two years of experience as a licensed practical nurse.
  • Capable of lifting up to 60 lbs. using proper techniques, capable of reaching for activities such as repositioning or to gain access to supplies, and is capable of pushing or pulling with sufficient strength.
Preferred Qualifications
  • Long-term care experience.
