Job Summary:

CVS Health is seeking a highly motivated and compassionate Retail Health Coordinator to join our team. As a key member of our healthcare team, you will play a vital role in delivering exceptional patient care and promoting healthy outcomes for our members.

Key Responsibilities:

• Coordinate patient flow and manage scheduling for appointments and tests

• Provide personalized support and guidance to patients and families, addressing their healthcare needs and concerns

• Collaborate with healthcare providers and other team members to ensure seamless care coordination

• Develop and implement patient care plans, incorporating evidence-based practices and patient preferences

• Communicate effectively with patients, families, and healthcare providers, using clear and empathetic language

Requirements:

• High school diploma or equivalent required; associate degree preferred

• Minimum 3-5 years of experience in healthcare, customer service, or patient relations

• Strong communication and interpersonal skills, with ability to work effectively in a team environment

• Proficiency in Microsoft Office applications, including Outlook, Word, and Excel

• Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities
