We are seeking a skilled Licensed Practical Nurse to join our team at Wellpath, where we provide quality care to underserved populations. Job Summary

The Licensed Practical Nurse plays a vital role in patient care by contributing to the nursing process of assessment, planning, implementation, and evaluation. Under the supervision of RNs, you will assess patients' physical, psychological, and social dimensions, contribute to individualized treatment planning, and implement treatment programs directed by Health Care Practitioners.

Key Responsibilities:

  • Assess patients' physical, psychological, and social dimensions, contributing to individualized treatment planning under RN supervision.
  • Implement treatment programs directed by Health Care Practitioners, administering medications, including IV therapy (if certified), and performing diagnostic tests according to established protocols and procedures.
  • Collaborate in controlled substance counts at the beginning of each shift, adhering to safety techniques, procedures, and standard precautions while responding to codes or health emergencies within standard guidelines.
  • Assist in medical or minor surgical procedures, implement nutrition and therapeutic diet plans, and document nursing encounters using SOAP charting, ensuring effective communication with nursing staff, physicians, and other relevant personnel.

Qualifications & Requirements:

To be successful in this role, you should have a strong foundation in nursing principles, excellent communication skills, and the ability to work effectively in a team environment. We require:

  • A graduate degree from an accredited school of nursing.
  • A minimum of one year clinic experience, preferably in a similar setting.
  • Current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment.
  • The ability to obtain and maintain CPR certification.
