Job Title: Healthcare Service Coordinator - Clinical Care Specialist

Salary: $65,000 - $85,000 per year

Job Overview

We are seeking a skilled Healthcare Service Coordinator - Clinical Care Specialist to join our team. In this role, you will work closely with individuals with chronic illnesses, co-morbidities, and/or disabilities, providing comprehensive care management services to optimize their healthcare outcomes.

Key Responsibilities:
  • Conduct face-to-face clinical assessments to identify member needs and develop individualized care plans
  • Coordinate care with healthcare teams, including physicians, specialists, and other providers
  • Supervise non-RN clinicians and direct their work in coordinating services for members
  • Review and revise care plans as needed to ensure optimal member outcomes
Requirements:
  • RN license in applicable state(s) required
  • Minimum 3 years of experience in healthcare service coordination or case management
  • Strong communication and interpersonal skills
  • Able to travel to worksite and other locations as necessary

Elevance Health operates in a Hybrid Workforce Strategy, requiring associates to work at an Elevance Health location at least once per week, and potentially several times per week. Specific requirements and expectations for time onsite will be discussed as part of the hiring process. Candidates must reside within 50 miles or 1-hour commute each way of a relevant Elevance Health location.
