Caregiver Advocate

We are looking for a dedicated and compassionate Caregiver Advocate to join our team at Agape Care Group. This role offers a unique opportunity to make a meaningful impact in the lives of patients and their families.
About the RoleAs a Caregiver Advocate, you will be responsible for providing emotional support and psychosocial evaluations to patients and their families during their end-of-life journey. You will work closely with our interdisciplinary team to develop and implement care plans that cater to the individual needs of each patient.
Key Responsibilities- Provide emotional support and counseling to patients and their families
- Conduct thorough psychosocial evaluations to identify patient needs and develop effective care plans
- Collaborate with the healthcare team to coordinate patient care and ensure seamless transitions
- Maintain accurate and up-to-date records of patient interactions and progress
To be successful in this role, you will need:
- A Master's degree in Social Work from an accredited institution
- A current state license as a social worker
- At least 2 years of clinical experience in healthcare or hospice care
- Excellent communication and interpersonal skills
- A strong ability to empathize with patients and their families
