The Licensed Practical/Vocational Nurse is a vital member of our healthcare team, responsible for providing direct patient care under the supervision of a Registered Nurse. Key responsibilities include:

Develop and implement patient care plans in collaboration with the interdisciplinary team.
Provide comprehensive nursing evaluations and assessments to ensure accurate and timely documentation.
Coordinate patient care and services, working closely with hospital social workers and community resources.
Utilize computer skills to document services in our electronic healthcare system.
Collaborate with medical professionals to advocate for patient health needs and build strong relationships.
Engage in effective transitions of care from inpatient/SNF settings to home or different levels of care.
Supervise field staff to ensure patient needs are met, and properly document and track activities for billing purposes.
Participate in the coordination of home health services and utilize equipment and supplies effectively.
Perform other duties as assigned by management.
