Job Description
As a Registered Nurse (RN) Complex Case Manager at Beebe Healthcare, you will play a vital role in providing expert case management services to medically complex inpatients. Your responsibilities will include:
• Conducting thorough assessments of patients' physical, functional, social, psychological, environmental, learning, and financial needs.
• Identifying problems, goals, and interventions designed to meet patients' needs, considering their preferences and desired level of involvement in the case management plan.
• Assisting with the creation of individualized care plans, including objectives, goals, and actions to meet patients' needs.
• Providing appropriate interventions that demonstrate knowledge of and sensitivity toward cultural diversity, religious, developmental, health literacy, and educational backgrounds of the population served.
• Assessing patients' formal and informal support systems, including caregiver resources and involvement, as well as available benefits and/or community resources.
• Implementing and monitoring individualized care plans to ensure their effectiveness and appropriateness, maintaining ongoing communication with UR Nurses regarding same.
• Evaluating patients' progress toward goal achievement, identifying and evaluating barriers to meeting or complying with case management plans, and systematically reassessing for changes in goals and/or health status.
• Researching alternative treatment options, selecting and locating appropriate providers, and facilitating referrals as needed.
• Communicating with attending and primary care physicians, as well as members of the comprehensive care team, regarding patients' status.
• Utilizing motivational interviewing skills to build patients' engagement in case management plans.
• Providing education, information, direction, and support related to care goals of patients.
• Acting as a patient advocate and assisting with problem-solving and addressing any barriers to care or compliance with care plans.
Required Skills and Qualifications
To be successful in this role, you must possess:
• A Bachelor's degree in nursing or a related field.
• OR an ADN with 5 years of case management experience, requiring BSN completion within 5 years of hire.
Credentials
You must hold an active RN license and have a Bachelor's degree in nursing.
