About the Role

Horizon Health is seeking a skilled Mental Health Therapist to join our team in Clive, Iowa. As a key member of our treatment team, you will provide high-quality patient care and play a vital role in helping individuals achieve their optimal level of emotional health.

Job Description

This opportunity offers a unique chance to work with a diverse range of patients and contribute to the development of personalized treatment plans. Our ideal candidate will possess excellent communication skills, empathy, and a strong understanding of mental health principles.

Responsibilities
  1. Provide individual, family, and group therapies within required timeframes
  2. Assess patient needs regularly and assist in developing and updating Master Treatment Plans
  3. Work collaboratively with the treatment team to ensure each treatment modality is appropriate for the patient
  4. Complete specialized clinical social work services, crisis intervention, and short-term and long-term family therapy as needed
  5. Maintain accurate and timely documentation consistent with policies and regulations
Requirements
  • Education: Master's degree from an accredited college or university in social work, counseling, or a related field
  • Licensure/Certification: Licensed in the state of Iowa as a Licensed Mental Health Counselor (LMHC) or Licensed Master Social Worker (LMSW)
  • Experience: One or more years' experience in a mental health setting preferred
