Job Title: LPN Home Health

We are seeking a skilled and compassionate LPN to join our Home Health team at WVU Medicine. As a LPN Home Health, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Job Summary:

The LPN Home Health will be responsible for providing direct and indirect nursing care to patients in their homes, under the supervision of an RN. This includes assessing patient needs, developing and implementing care plans, and evaluating patient outcomes.

Responsibilities:
  • Provide basic patient care according to policies and procedures
  • Use the nursing process to meet patient physical and psychological needs
  • Perform and monitor patient teaching according to policy and procedures
  • Coordinate care to ensure medications are administered as ordered by a physician
  • Communicate patient care problems or potential problems to the primary RN and Administrative staff
  • Maintain patient confidentiality according to policies and procedures
  • Function effectively as a LPN in respect to organization, communication, and prioritization
Requirements:
  • Graduate of an LPN school
  • Current unencumbered licensure with the WV Board of Practical Nursing, or appropriate state board where services will be provided, as a practical nurse
  • Must possess valid WV or neighboring state driver's license and must maintain WV or neighboring state minimum auto insurance
  • Current CPR card
Preferred Qualifications:
  • One year of Home Care Nursing preferred

  • Exposure to wind, rain, snow, sleet, mud, high and low temperatures, animals and insects, and hazardous roadways
Skills and Abilities:
  • RN performs the initial assessment upon admission and the LPN does continuing assessments, thereafter, implementing and evaluating with the RN patient care, according to policy using judgment to determine the physical, psychological and social needs of the patient and documents this in the chart
  • Demonstrates competency in technical and clinical skills, completes skills checklist, and correctly performs these skills daily as required
  • Administers medications by all acceptable routes as ordered by physician and authorized by RN according to accepted policy and procedure
  • Communicates plan of care and patient needs effectively, in a multi-disciplinary fashion to any and all involved persons and/or departments
