Job Responsibilities

  • Develop and maintain trusting relationships with clients to ensure they receive the necessary support for success.
  • Provide Health Home Care coordination services for children with low to high acuity needs, including up to two services per month, with at least one face-to-face interaction.
  • Offer education to families and children regarding service options, conditions being treated, and desired effects.
  • Document all case activity, including persistent outreach, consent development, assessments, crisis supports, plan of care development, client progress, and transition arrangements.
  • Administer the CANS-NY assessment annually, updating as needed.
  • Inventory and coordinate existing services relevant to the Plan of Care, identifying and securing additional services when necessary.
  • Convene and conduct yearly meetings to review progress, update POCs, and confirm continuing client eligibility.
  • Ensure responsible transition of client service into and out of Health Home care, between child and adult health homes, and between inpatient and community care as required.
  • Ability to travel to surrounding counties to meet with clients and families.
  • Assist families and children in overcoming barriers such as transportation and acquiring public benefits.

Requirements

  • Bachelor's degree with a minimum of two years' experience; or Master's degree with a minimum of one year's experience; or Registered Nurse with a minimum of two years' experience.
  • Relevant experience in serving children and families in child welfare, developmental disabilities, behavioral health, primary health care, or social services.
  • Experience coordinating and participating in team settings.
  • Ability to deliver family-driven, youth-guided services.
  • Vehicle and a valid NYS driver's license required.
