Overview

Chestnut Health Systems is a leading provider of behavioral and primary healthcare services. As a Direct Care Staff, you will play a vital role in supporting individuals with substance use and mental health issues.

Estimated Salary: $38,000 - $45,000 per yearJob Description
  • Complete required data entry and paperwork in accordance with program and agency policies and procedures, including but not limited to LOCUS, Health Risk Assessment/Physical Health Assessment (HRA/PHA), IM-CANS, progress notes, and other assigned requirements.
  • Provide services identified in the IM-CANS to meet consumer needs that are consumer driven and meet the program requirements, including maintaining a minimum of assigned consumer service levels, completing pre-authorizations and re-authorizations of services, providing services primarily to include IATP, therapy, community support, medication training and monitoring, and linking to additional clinical resources when needed.
  • Promote the recovery model of treatment, including trauma-informed care, hope, respect, empowerment, health/wellness, and spirituality/connectedness by assisting consumers with regaining control over their own lives through self-directed recovery.
  • Utilize evidenced-based practices focusing on skill-building and individual, family, and group therapy.
Requirements
  • Must be 21 years of age and have a high school diploma or equivalent and 6 months peer or social service experience.
  • Requires basic computer skills and experience to include MS Word, MS Excel, Outlook, Adobe Acrobat, and an electronic medical record (EMR).
  • Satisfactory completion of Chestnut's background checking process.
